***If applicant for Passport Card already holds a valid passport book, do not submit $25 Execution Fee.

2.) 2 Color passport pictures (2" x 2")	2.) 2 fotografías a color tipo pasaporte (2" x 2")
3.) Certified birth certificate (long form) or Certificate of Naturalization	3.) Acta de nacimiento (en forma larga) ó Certificado de Naturalización
4.) 2 Money Orders (for each application)	4.) 2 Giros Postales (para cada solicitud)
5.) Valid ID or Driver's License for adults/parents	5.) Identificación ó licencia para adultos/padres
